Root Cause Analysis

The vulnerability stemmed from missing authentication attempt tracking in two key functions:

Authenticate() processed login requests without any rate-limiting checks (added via loginAttemptCache.Allow() in the patch)
lookupConsoleUser() performed password validation without tracking failed attempts (patched with loginAttemptCache.Add() calls) The absence of these protections in the original code allowed unlimited brute-force attempts. The patch introduced a new LoginAttemptCache and integrated it into these authentication flows, confirming these were the vulnerable points.
